Pioderma gangrenoso: Revisión de la literatura y estudio transversal retrospectivo en el Hospital Universitario de Basurto

[ES] Introducción. El pioderma gangrenoso (PG) es una dermatosis neutrofílica autoinflamatoria que se caracteriza por una úlcera dolorosa de base purulenta con bordes socavados y necróticos. La ausencia de criterios diagnósticos, pruebas de laboratorio y hallazgos histológicos específicos hacen de su diagnóstico un reto. La respuesta al tratamiento tópico no siempre resulta favorable, por lo que, en ocasiones, se requerirán tratamientos inmunosupresores agresivos durante largo tiempo.
Objetivos. Revisar la evidencia científica existente, y analizar los datos de 18 pacientes con PG atendidos el Hospital Universitario de Basurto (HUB). Posteriormente, comparar los hallazgos de la literatura con la realidad hospitalaria.
Material y métodos. Se investigó en PubMed, Cochrane, Web of science, Up to date y Scopus. Se accedió a las historias clínicas mediante Clinic y Osabide global. Se completó una base de datos que fue analizada con los programas SPSS y STATA.
Resultados y discusión. Se ha descrito la historia, epidemiología, fisiopatología, clínica, diagnóstico, anatomía patológica, tratamiento, pronóstico, calidad de vida y prevención del PG, destacando su complicado manejo y las terapias futuras. Además, se ha observado similitud entre los hallazgos de este estudio y la evidencia científica existente, así como alguna asociación significativa entre las diferentes variables.
Conclusiones. Se cree que el PG tiene un origen multifactorial basado en la disfunción de neutrófilos y en alteraciones genéticas e inflamatorias. Su diagnóstico es clínico y de exclusión. La evidencia sobre el manejo es limitada al no existir ensayos clínicos controlados y aleatorizados, siendo necesarios más estudios. Se espera que las nuevas terapias emergentes mejoren su manejo. [EN] Introduction. Pyoderma gangrenosum (PG) is an autoinflammatory neutrophilic dermatosis characterized by a painful ulcer with a purulent base, and undermined and necrotic edges. The lack of diagnostic criteria, laboratory tests and specific histological findings make its diagnosis a challenge. The response to topical treatment is not always favorable, so, in many cases, aggressive treatments will be required for long time.
Objectives. To review the current scientific evidence, and to analyze the data of 18 patients with PG treated in the University Hospital of Basurto (HUB). Subsequently, compare the findings of the literature with the hospital reality.
Material and methods. Pubmed, Cochrane, Web of science, Up to date, and Scopus were used. Medical records were accessed through Clinic and Osabide Global. A database was completed and analysed using SPSS and STATA programmes.
Results and discussion. The history, epidemiology, physiopathology, signs/symptoms, diagnosis, pathological anatomy, treatment, prognosis, quality of life and prevention of PG have been described, highlighting its complicated management and future therapies. In addition, similarity has been observed between the findings of this study and the existing evidence, as well as some significant association between the different variables.
Conclusions. PG is believed to have a multifactorial origin based on neutrophil dysfunction as well as on genetic and inflammatory variations. Its diagnosis is clinical and by exclusion. The evidence for management is limited in the absence of randomized controlled clinical trials, so further studies are needed. New emerging therapies are expected to improve its treatment.
